We participate with many local and national insurance plans, but it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive. If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. However, depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or deductible amounts. You are responsible for paying those charges.
If your insurance requires a co-payment, we expect to collect that at the time of service. If you do not pay your co-payment at the time of service, there will be a $15 billing fee added to your charge. Your insurance will not cover this fee.
If your insurance requires pre-approval or referrals to our practice, it is your responsibility to make sure that referral has been made. If you request to be seen before a referral has been approved, you may do so by signing a release wherein you agree to pay for the visit if a referral is not approved.
Many insurance plans require that patients obtain laboratory, diagnostic or hospital services (such as blood work, CT scans, or MRIs) at specific labs or hospitals. You are responsible for choosing facilities that are covered by your insurance, or you will be responsible for any charges that are not covered.
We will complete one set of insurance forms for you at no charge. Additional insurance forms will be billed at $10 per form. Please allow 7-10 business days for completion of disability, compensation or insurance forms.
If you do not have health insurance, you will be asked to pay for services prior to your appointment with our doctors, or to arrange a payment plan with our billing staff prior to your visit. We accept cash, checks, credit and debit cards.
If your insurance coverage has terminated, or you have provided incorrect information, you are responsible for paying for services we provide to you. If we are unable to collect these bills from you, we may use a collection agency to recover payments. If this occurs, you will be responsible for all collection charges and fees associated with your account.
Please talk with our staff if you have any questions about your services, your insurance coverage, or your personal financial situation. We want to work with you to avoid any financial problems and to assure that your account remains in good standing.
